The Registered Nurse (RN) in Primary Care provides support to the VAPAHCS Environmental Medicine team which may provide Telehealth and face-to-face services to all within VAPAHCS. Services provided by VAPAHCS Environmental Medicine staff are conducted via multiple modalities, including virtual and in-person care. The RN position is organizationally located in the VAPAHCS Environmental Medicine and Compensation and Pension (C&P section) within Integrated Primary Care Service

Duties include but not limited to the following: Responsible for providing direct patient care to Veterans and training and support to VAPAHCS Environmental Medicine staff as appropriate. Provides professional assessment, coordination, telephone triage and planning of multiple health care services. Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Demonstrate the ability to cope with and manage competing priorities. Self-directs in goal setting for managing complex client situations. Demonstrate proficiency in practice based on conscious and deliberate planning. Coordinates scheduling of appropriate labs and appointments. Performs screenings, visit triage, and care coordination including COVID related testing, lodging and transportation to and from the VAPAHCS, and coordination of environmental exams and follow up care if needed. Triages EH consults and updating Environmental Health registries. Provides patient and family health education with a focus on the TE screening process, understanding various Toxic Exposures and symptoms, relevant disease self-management, prevention, and wellness.
