The Outpatient Specialty Clinic RN Navigator focuses on the coordination, integration and direct delivery of services to failitate achievement of quality cost effective outcomes by working collaboratively with specialty interdisciplinary staff.

  • Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Uses the nursing process and evidence-based practice to collaborate with the team, in developing a patient-driven holistic plan of care. Focuses on the coordination, integration and direct delivery of services to facilitate achievement of quality cost effective outcomes by working collaboratively with specialty interdisciplinary staff; participating in quality improvement processes related to patient care; emphasizing early assessment and intervention, comprehensive care planning and inclusive referrals to all internal and external disciplines who may impact the care; and monitoring patient activity and resource distribution.
