The Patient Aligned Care Team (PACT) Coordinator supports the implementation and sustainment of the PACT model within Primary Care/Ambulatory Care at the Dublin VA Medical Center, providing education and training to PACT staff, coordinating communication and workflows, supporting compliance with VA standards, and promoting high-quality, Veteran-centered care through interdisciplinary collaboration.

• Duties The Patient Aligned Care Team (PACT) Coordinator duties include, but are not limited to: Serves as a Primary Care PACT champion, supporting the implementation, coordination, and sustainment of the PACT model within the Ambulatory Care setting. Provides education and training to PACT team members regarding PACT principles, roles, responsibilities, and the model of care. Assists PACT teams with organizing care delivery and monitors adherence to the established PACT model of care. Identifies and assists in resolving communication, workflow, and operational challenges affecting PACT teams and collaborates with leadership and other stakeholders. Supports PACT teams in meeting VACO, VISN, and local policies, directives, Primary Care standards, clinical practice guidelines, and clinical pathways. Participates in the development, implementation, and evaluation of interdisciplinary, Veteran-centered care and promotes Whole Health, patient education, self-management, and customer satisfaction. Assists with initiatives to improve clinic systems, processes, patient experience, and quality of care. Supports quality and performance improvement activities, including identifying opportunities for improvement, monitoring PACT outcomes and benchmarks, and assisting with improvement strategies. Collects and communicates data related to daily PACT operations and reports progress toward established performance and clinical benchmarks. Maintains professional collaboration with interdisciplinary team members and provides coaching and support to PACT staff regarding standards of care and the PACT model.
