The primary purpose of the Licensed Practical Nurse assigned to PACT/Medical Specialty clinics are held accountable for delivering direct patient care to adult through geriatric Veterans for both acute and chronic diseases in an outpatient clinic setting. The LPN recognizes age related needs and demonstrates knowledge and care appropriate to those age groups.

The LPN interfaces with a variety of VA and community providers directly, over the phone, through correspondence, and electronically via the computer, fax modality, and teleconferencing. The LPN participates in a variety of quality improvement functions as a vital team member of the Illiana Health Care System. Duties include, but are not limited to: Observes and reports changes in patients' physical and emotional conditions utilizing elements of the nursing process of signs and symptoms identification, observation, perception, clinical measurements, screening protocols, and prescribed history taking Reporting mechanisms include standardized electronic and manual documentation, with a major emphasis on computer data entry, as well as telephone, fax, and oral communication with community and VA providers and their support Services Screens, intervenes, and follows through for acute/chronic medical and emotional conditions for adult through geriatric populations with minimal supervision of a registered nurse/provider Effectively demonstrates ability to gather data and observations, assist in planning, implementing, documenting and evaluating the care of patients seen in the outpatient care setting Distinguishes between normal changes that accompany the aging process vs. pathological changes Recognizes priorities and adjusts patient care accordingly Recognizes and intervenes appropriately for urgent and emergency patient needs Confers with other providers and Services to meet patients' needs beyond LPN scope of practice and experience Demonstrates clinical competence in performing core and practice specific procedures according to the Competency Checklist Work Schedule: 7:30am - 4:00pm; Monday-Friday Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases when setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience. Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
