This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. • *** Recruitment or Relocation incentive may be authorized for eligible, highly qualified candidates! *** The Community Living Center (CLC) Registered Nurse (RN) executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex resident care. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for a group of residents. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through knowledge sharing to provide safe, quality nursing care. Additional duties include, but are not limited to: Assesses, plans, implements, and evaluates care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex client care. Fosters a safe and supportive environment conducive to the professional development of health care professionals. Evaluates outcomes of evidence-based decisions and practice changes for individuals, groups, and populations. Responsible for the documented outcomes at the program or service level. The CLC RN collaborates with the Minimum Data Set (MDS) coordinator to ensure accuracy of the Resource Utilization Group (RUG) score. Has oversight for admission/approval process for nursing home patients. Responsible for providing Nursing Care to Long Term care residents, dementia unit residents, respite patients, hospice patients, rehabilitation patients, transitional care patients and their families/significant others. Have knowledge and expertise in areas of: Pain management, Palliative Care, RAI/MDS, Accu-max software, Meds and Rx via BCMA, Geriatric Assessment Skills, Care of Dementia Patients, Physical Restraint Policy & Procedure, Discharge Planning and Teaching, Facilitate Inter-disciplinary Meetings, End of Life Care Planning, Rehabilitative Nursing. Additional duties as assigned.
