This position is located at the Fort Thompson Health Center in Fort Thompson, SD. The primary purpose is to independently plan and execute case management activities, coordinates care for patients with complex acute and chronic health conditions, precedent-setting case management situations, and serves as a clinical resource to lower-graded nursing staff on case management and discharge planning matters. A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).
- Coordinates care across providers, disciplines, and care settings to ensure continuity and prevent fragmentation of services. Provides transition of care for patients with complex clinical and social needs, designing individualized transition plans that address nonroutine discharge barriers and ensure safe, effective handoffs across the continuum of care. Provides individualized education to patients and families regarding disease processes, medications, treatment plans, self-management strategies, and post-discharge care requirements using established educational materials. Provides direct professional nursing care to assigned patients with chronic and recurring health conditions.
