Licensed Practical Nurse - Ambulatory Care

The Licensed Practical Nurse (LPN) serves as a member of the multidisciplinary health care team with selected patients and provides care to them on a regular and recurring basis. The LPN completes assigned duties and responsibilities, which involve performing standard and specialized practical nursing activities and which require broad work experience. Recurring duties are carried out independently. The LPN recognizes the need for and seeks guidance when necessary to ensure quality patient care

• The Licensed Practical Nurse provides direct nursing care to patients/residents in a variety of health care settings, including but not limited to: Observing changes in patient's/resident's condition, (e.g. behavior shifts, mood changes skin coloring, appetite, etc). Documenting care provided and observations on appropriate tools or forms. Assuring receipt of adequate nutritional intake either through observation, feeding, or properly positioning and checking placement of the feeding tube for the patient/resident. Administering medications, stoma care (e.g. tracheotomy, suctioning, colostomy), enemas, skin care, simple dressings, braces and prosthetic devices. Monitors intake and output, according to established policy and procedure. Recognizing the patient's/resident's autonomy and promoting independence by means of respect, limits setting, and individualized treatment. Reinforcing age related healthcare education for patients/residents and or significant other. Independently performs EKGs for unit patients operating a standard 12 lead EKG machine. Makes voltage adjustments when indicated. Repeats EKGs or repositions patients to eliminate artifact, especially with Spinal Cord Injury patients. Makes notation on EKG report for provider when a patient has a pacemaker or an AICD unit. Integrate and apply pertinent aspects of the Outpatient Clinic performance improvement program into his/her professional practice as applicable. Schedule appointments for patients in collaboration with team members to ensure that all patients are scheduled within time constraints established by ACA guidelines and VHA policies especially adhering to priority access for service connected and OEF/OIF veteran populations. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
