Public Health Nurse - Family Health

Carver County is looking for a Public Health Nurse who wants to make a meaningful impact on community well-being every day. Under the direction of the Family Health Supervisor, the Public Health Nurse (PHN) promotes and protects the health of the Carver County residents by delivering essential public health services grounded in the Foundational Public Health Services Framework. This role combines clinical expertise with systems-level thinking to address health equity, prevent disease, and improve population health. The PHN provides direct services and systems-level interventions across a range of public health domains including, but not limited to infectious disease control, immunizations, tuberculosis case management, maternal/child health, refugee health, emergency preparedness and response, and resource navigation.

Typical duties may include some of the following:

Infectious Disease Control

• Conduct case investigations and contact tracing for reportable diseases
• Provide education and guidance to affected individuals and communities
• Collaborate with healthcare providers, schools, and MDH to contain outbreaks

Vaccination Services

• Administer immunizations in clinics, schools, and community settings
• Educate the public on vaccine-preventable diseases and immunization schedules

Tuberculosis Case Management

• Coordinate screening, treatment, and follow-up for active and latent TB cases
• Monitor treatment adherence and side effects
• Serve as liaison between clients, providers, and public health labs

Maternal/Child Health Services

• Implement evidence-based family home visiting models to support healthy pregnancy outcomes, child development, and caregiver well-being
• Promote and facilitate participation in the Follow Along Program and refer to early intervention systems (e.g., Help Me Grow)

Resource Navigation & Health Equity

• Connect individuals and families to housing, food, mental health, and social services
• Advocate for culturally responsive care and equitable access
• Identify service gaps and contribute to policy and program development

Population Health & Systems Work

• Assess community health needs using data and informatics
• Design and evaluate evidence-based interventions
• Collaborate with schools, healthcare, community partners, and public health colleagues
• Participate in emergency preparedness and response activities

• Registered Nurse's License with Public Health Nurse Certification in the State of Minnesota
• Two years paid work experience as a four year RN and/or PHN
• Basic Cardiac Life Support Certification
• Valid Minnesota driver's license
or
•Equivalent combination of education and experience necessary to perform the essential functions of the position

PREFERRED EXPERIENCE BEYOND MINIMUM QUALIFICATIONS:
• Experience in maternal/child health, infectious disease, emergency preparedness and response, working with families and children with special health conditions and/or adverse childhood experiences, and history working with evidence-based home visiting models (e.g., MECSH).
• Experience with contact investigation and contact tracing and/or other work related to infectious disease prevention, intervention, mitigation and recovery. 
• Fluent in Spanish

If position is assigned to the Home & Community Based Care department, candidate must also successfully complete and maintain eligibility to provide direct care to clients in accordance with Minnesota Statutes §245C.03All applications will be screened for minimum qualifications, scored and ranked. The highest ranked candidates meeting minimum qualifications will be recommended for the scored oral examinations (interviews). Final selection for the position will include successful completion of oral interview(s), reference checks, background investigation, and pre-placement health screening.
