About the Position: This position is located at Naval Hospital Jacksonville. This is a Direct Hire Solicitation. Salary negotiation may be available for those candidates who are new to Federal service. Incentives may be available for highly qualified applicants.
- Coordinates services needed by patients with a variety of complexities and provides intervention when the patient deviates from the anticipated path of progression.
- Conducts a comprehensive assessment of beneficiary’s health needs to develop a plan of care.
- Serves as liaison between the patient and the health care professionals.
- Conduct assessment and identify needs to formulate care plans for patients with a variety of complexhealth concerns, which includes researching, analyzing, testing, and evaluating information, uncommon situations, or unpredictable conditions.
- Coordinates services, referrals, and appointments for each patient under case management services.
