About the Position: This position is located at the Naval Branch Health Clinic Belle Chasse, Dir of Medicine. Play an integral part in the care for a wide range of medically complex or seriously ill/injured patients in their case management and care coordination. Candidate will work in an office in the medical building located on NAS JRB New Orleans. Hours are from 0700-1530 with a 30 minute lunch. This is a Direct Hire Solicitation utilizing DHA for Certain Personnel of the DoD Workforce.
- Uses medical knowledge to formulate a plan and present to provider.
- Serves as liaison between the patient and the health care professionals.
- Assess patients with various complex medical conditions daily.
- Attends daily multi-disciplinary team meetings to address current issues, and improve the overall treatment process.
- Conducts a comprehensive assessment of beneficiary’s health needs to develop a plan of care.
