This position is located at the Lawton Indian Hospital. Incumbent is responsible for all aspects of Case Management for all patients. Coordinates available resources by identifying, organizing, and utilizing resources to meet the patient's needs. This position is supervised by the Supervisory Nurse (Case Manager). A REAL ID will be required beginning May 7, 2025, in accordance with 6 C.F.R. 37.5 (2021).

Coordinates referrals, transitions, and services across healthcare settings; collaborates with interdisciplinary teams; identifies care gaps and barriers; and contributes to improvements in case management and care coordination processes. Provides professional nursing care, clinical assessments, interventions, treatments, follow-up, and patient/family education to promote disease management, treatment adherence, self-management, and patient safety.
