Community Health Nurse

This position focuses on assessing local health needs, planning community programs, and delivering direct public health services within Berks County. The work supports statewide health priorities through education, outreach, surveillance, and partnership-building. As a Community Health Nurse, you will perform the following duties:

• Community Assessment: Review county health data to identify needs and guide public health activities
• Program Development: Create initiatives based on protocols, guidelines, and community priorities
• Health Monitoring: Track communicable diseases and enter data into state health systems
• Public Education: Provide information on health topics to support community awareness
• Outreach Services: Offer case management and support to individuals and targeted groups
• Clinical Care: Deliver immunizations and conduct disease-related specimen collection according to guidelines

Work Schedule and Additional Information:

• Full-time employment, 37.5 hours per week
• Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch.
• Work infrequently requires statewide travel and irregular or flexible work hours, including overnights, weekends, and holidays, in accordance with policy.
• Telework: You will not have the option to telework in this position.

QUALIFICATIONS
Special Requirements:

• This position requires active authorization to practice as a Registered Nurse in Pennsylvania.
• All employees possessing an active temporary practice permit must obtain licensure as a Registered Nurse within the one (1) year period defined by the Pennsylvania State Board of Nursing

• You must be able to perform essential job functions.

Legal Requirements:

• You must pass a background investigation.
• This position falls under the provisions of the Child Protective Services Law.
  • Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
