The Women's Health (WH) Clinical Navigator Registered Nurse is responsible for providing competent, evidence-based care to assigned patients. This nurse has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. Provides patient management, care coordination, and disposition planning for Veterans.

• The Women's Health (WH) Clinical Navigator Registered Nurse is responsible for providing competent, evidence-based care to assigned patients. This nurse has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. Provides patient management, care coordination, and disposition planning for Veterans, Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Coordinates care with multiple providers across all levels and sites of care, including but not limited to: The Women's Health (WH) Clinical Navigator Registered Nurse provides oversight of the patient's journey through the diagnostic and treatment continuum, ensuring that all steps in the process are scheduled, timely and completed, while tracking the plan of care. The Women's Health (WH) Clinical Navigator Registered Nurse coordinates and facilitates, as needed, WH care including contacting the Veteran, facilitating appointments and authorizations, tracking receipt of test results and other medical documentation, and notifying providers of needed follow up care. The WH Clinical Navigator Nurse will interface with outside referral agencies and ensure that their business and clinical practices follow regulatory standards. Anticipates and implements preventive patient care measures. The WH Clinical Navigator Nurse provides guidance and education to patients and caregivers and collaborates with other professionals at all levels. The WHCN - Maternity makes regular telephone contact as outlined in VHA Directive 1330.03 Maternity Health Care and Coordination, the Maternity Care Coordination Guidebook, and based on the Veteran's need while the Veteran is in the care of the VA authorized provider in the community and documents the call(s) in the electronic health record (EHR). Informs the Veteran of VA's authority to furnish newborn care and the need for other. The WH Clinical Navigator Nurse provides patient management, care coordination, and disposition planning for Veterans, limited to addressing mammography, cervical cancer screening, bone density testing, sexually transmitted disease testing and treatment, maternity care, reproductive health, and other women's health services. The WH Clinical Navigator Nurse assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. The WH Clinical Navigator Nurse coordinates care with multiple providers across all levels and sites of care.
