The Registered Nurse (RN) will demonstrate proficiency in practice based on conscious and deliberate planning. Self-directed in goal setting for managing complex client situations. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences outcomes by collaborating with members of the interdisciplinary team.

  • Duties include but are not limited to: The Registered Nurse (RN) Patient Aligned Care Team (PACT) Outpatient Staff is responsible for providing competent, evidence-based care to primary care patients with oversight of licensed vocational nurses/nursing assistants as appropriate to the setting. This nurse provides direct Veteran care and care coordination, as it pertains, to Veterans and their care. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. This nurse provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. The nurse will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and enhance outcomes utilization. This nurse integrates outpatient nursing knowledge, skills, abilities, and judgment and is self-directed in goal setting for managing complex PACT situations. Validates understanding of the rationale for care processes and decisions. Questions clinical practices for the purpose of providing evidence-based care. The nurse will demonstrate the ability to cope with and manage competing priorities. The nurse demonstrates competence in primary care practice and decision-making, deliberate planning, and critical thinking skills. Delivers self-directed, professional nursing practice using the nursing process (assessment, diagnosis, outcome identification, planning, implementation, and evaluation) for complex patients based on conscious and deliberate planning with occasional supervision. 1. Independently plans and delivers technically competent care to patients with complex needs in alignment with the ANA Standards of Nursing Practice. 2. Manages the care of patients, while organizing and prioritizing patient care needs, and ensuring assigned tasks are completed. 3. Functions as an emerging leader in providing direction to staff to identify, implement and evaluate patient care needs. 4. Collaborates with the interdisciplinary team to ensure the plan of care is achieved. Potential TODs include: 0600-1430 (Monday through Friday) 0730-1600 (Monday through Friday) 0930-1800 (Monday through Friday) Please note: Compressed tours are not an option for this position currently.
