The Home Based Primary Care Nurse NP is a professional, independent practitioner with advanced education and appropriate experience who provides evidence-based care for a specific population of patients. The NP uses advanced assessment, planning and evaluation, as well as approved standardized procedures, to provide preventive and illness management healthcare incorporating the patient's age, gender, culture and learning abilities in the treatment plan.
- The Advanced Practice Nurse (APN) II Certified Nurse Practitioner (NP) practices in Community Outpatient Services (COPS)/Home Based Primary Care (HBPC) Program to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. The NP is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties and responsibilities of the position Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. Develops and implements an individualized plan to achieve optimal population health and disease management. Demonstrates leadership and works collaboratively with the interdisciplinary team. Order and interprets labs, tests, and diagnostic studies. Prescribes pharmacologic and non-pharmacologic interventions. Initiate referrals and consultations to specialty services. Provides education to patients and their families. Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes.
