The Registered Nurse (RN) - Home Based Primary Care (HBPC) provides comprehensive and coordinated nursing care to an assigned panel of patients. The RN in this role collaborates with Veteran Affairs (VA) and community services, as appropriate, to effectively meet the health promotion or disease prevention, acute, chronic, and long-term needs, based on Veterans' goals and plan of care with a focus on self-management. This position requires travel within the community and to Veterans' homes.

• The incumbent of this position must have a valid, state-issued driver's license. Registered Nurse (RN) - Home Based Primary Care (HBPC) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The RN in this role assumes responsibility for the coordination of care focused on patient education, self management, and customer satisfaction throughout the continuum of care. The RN - HBPC is also responsible for the following duties: Administering medications and procedures per established policies and guidelines. Influencing care outcomes by collaborating with members of the interdisciplinary team. Providing competent, evidence-based care to HBPC patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. Providing direct Veteran care and care coordination, as it pertains, to Veterans and their care within the HBPC Program. Being responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. Planning and delivering technically competent care in the home of Veterans with complex needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. Utilizing interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room visits, per Veteran's goals of care. Making home visits to Veterans in the program. Assisting in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Demonstrating leadership in delivering and improving holistic, home-based care through collaborative strategies with others. Contributing professional outpatient nursing perspective in discussions with the interdisciplinary team. Partnering with others to effect change and produce optimal outcomes. Supporting colleagues through knowledge sharing to provide safe, quality HBPC nursing.
