The Nurse Health Promotion and Disease Prevention Program Manager Registered Nurse is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Provides care and counseling of persons or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment and evaluation of human responses.

• The Nurse II Health Promotion and Disease Prevention (HPDP) Program Manager (PM) Registered Nurse (RN) executes position responsibilities that demonstrates leadership, experience, and creative approaches to the management of program aspects. The HPDP PM Registered Nurse demonstrates performance and leadership that is broad enough to impact the standard of care and safety of staff while caring for all patients throughout the continuum of care. Other duties include and are not limited to the following: Advises facility and primary care leadership on clinical and administrative issues related to HPDP, including clinical preventive services. Advocates, leads, or participates in policy, program development, and resource allocation to support HPDP programs and services for Veterans. Demonstrates the ability to improve the effectiveness of communication and interactions with others, to inspire their cooperation and confidence to ensure safe and effective outcomes in the current practice setting. Leads the effort through data collection and analysis to identify HPDP challenges, priorities for improvement, and opportunities for disseminating and sustaining best practices. Primary point of contact for national health promotion campaigns and events, ensuring local implementation and participation. Coordinates the development of user-friendly tools to assist in successful implementation of HPDP programs and services. Collaborates with community partnerships and resources, and Whole Health. Provides leadership in the development or modification of service delivery processes to support the successful implementation of HPDP services. Responsible for leading a facility wide program for all services within the organization that meets the requirements of National Directives. Oversee the provision of staff education, orientation, competencies, and providing quality improvement consultation related to the program specifically. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Provides peers with informal constructive feedback for improvement. Contributes professional nursing perspective in discussions with the interdisciplinary team. Improves Veteran/patient driven care by resolving experience issues such as coordination of care, timeliness or process issues, common themes of dissatisfaction, or environment of care concerns for the immediate practice setting Participates in the advancement of the profession through mentoring, precepting, and/or teaching. Participates in activities and strategies to sustain evidence-based-practice. Continuously evaluates the program, collecting and submitting facility performance measures. Leads patient care improvement through monitoring, analyzing, and evaluating care outcomes in the immediate practice setting. Consistently communicates and treats customers (patients, visitors, volunteers, and all Medical Center staff) in a courteous, tactful, and respectful manner.
