The LTC Charles S. Kettles VA Medical Center is seeking a Full-Time Registered Nurse (RN) - Falls Prevention Program Manager, aligned under Patient Care Services.
- The Falls Prevention Program is based on national standards, evidenced-based practice, and outcomes research in accordance with the scope of practice. Primary responsibilities for this position include systematic evaluation of current practice, development, coordination, and management of the Falls Prevention Program throughout the facility. Serves as a consultant for problem resolution across service/discipline lines. Maintains flexibility with a high degree of interpersonal skills, effective communication, leadership, motivation, self-direction, and ongoing evaluation of activities. Utilizes the nursing process in assessment, planning, implementation, and evaluation of quality and safe care of Veterans evidenced by comprehensive management of patient safety standards and best practices. Follows procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. The Falls Prevention Program Manager (PM) Registered Nurse (RN) focuses on individual growth and development of a Falls Prevention Program in practice with increased levels of self-direction and responsiveness. The nurse brings basic Falls Prevention principles and theory with application of critical thinking skills and implementation ability. Responsible for producing and disseminating an evidenced-based, cost-effective Fall Prevention Program based upon the patient population. Making fall prevention equipment and supply recommendations. Coaching unit managers and their staff on fall prevention strategies. Establishing and maintaining a facility Fall Prevention policy and fall clinical practice guidelines based on evidence. Developing falls prevention education, collaborating with the interdisciplinary team. Developing patient/caregiver education programs for fall and injury prevention. Overseeing and managing the local falls database. Formulating trends and providing the trends at least annually to the Associate Director of Patient Care Service (ADPCS). Establishing benchmarks. Assisting and leading Fall Root Cause Analysis (RCA) and annual fall prevention program aggregates. Submitting Fall Program reports, such as annual aggregate studies, Patient Safety Assessment Tool (PSATs), or RCAs, as required. Working in collaboration with the Patient Safety Managers (PSMs) and the staff to build a culture of safety.
