Chief Nurse - Community Care Service

This position is located at the Wilmington VA Medical Center Wilmington, DE. The Nurse IV Chief Nurse (CN), Community Care Service, executes leadership that is characterized by substantial and continuous responsibility and accountability for population groups or integrated programs that cross services and/or discipline lines and influence the organizational healthcare mission.

• Major duties include, but may not be limited to: Providing senior leadership and oversight for daily clinical and administrative operations within the Integrated Veteran Care as directed by the Chief of Staff. Responsible for all aspects of care that fall under the community care coordination service line, providing administrative oversight and direction to the programs within the service. Ensuring the delivery of coordinated care to Veteran patients and managing personnel within referred care programs to include community care. Collaborates with and provides leadership in the assessment, development, implementation, and evaluation of comprehensive coordination of care programs. Responsible for efficient and effective management of resources within Integrated Veteran Care. Responsible for assessing and recommending the allocations of human and material resources, in determining staffing levels, managing overtime, and recruitment and retention of nursing and administrative staff. Possesses solid knowledge of Integrated Veteran Care regulations, Veterans Health Administration (VHA) guidelines, clinical review criteria, clinical practice and documentation requirements, current standards of care, and compliance guidelines. Supports the integration and collaboration of nursing with other professional disciplines and functional areas in the mutual achievement of patient-centered and organizational goals. Champion a High Reliability Organization (HRO) of safety founded on a systems approach to developing evidence-based health care solutions based on prevention, not punishment, with collective mindfulness. Fosters a climate for practice that enhances job satisfaction through mutual responsibility for productivity and quality improvement. Collaborates with interdisciplinary team members, including other program and service leaders at the facility, network and program office levels, meet the needs of the Veteran, and result in positive clinical and administrative outcomes. Relates effectively with customers, families, and the community as well as program and service leaders at the local, network, facility and program office levels. They assist with healthcare operations and the functions and activities expected of them. Contributes ideas and recommendations for the establishment of standards of care, policies, and objectives for the enhancement of care. Ensures the Integrated Veteran Care program follows statutory law, accrediting organizations, and Veterans Health Administration (VHA) regulations, policies, and procedures. Performs other duties as assigned.
