The VA Northeast Ohio Healthcare System is recruiting for a Social Worker with the Behavioral Health Interdisciplinary Program (BHIP). The incumbent functions as a member of the BHIP team in providing for the biopsychosocial assessment, DSM diagnosis, and treatment of veterans who requirebehavioral health interventions.
- The Social Worker duties include, but are not limited to: Performs case finding functions and outreach, identifying and screening patients for psychosocial needs. Conducts psychosocial assessments and develops treatment plans in collaboration with the veteran/family and with the BHIP treatment team. Coordinates community-based services, including information and referral for additional services from other VA programs, other government programs and community agency programs. Assess and identifies high-risk veterans and follow protocol for suicide prevention. Utilizes principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Provides consultation services to other staff regarding the psychosocial needs of veterans and the impact of psychosocial problems on health care and adherence to the treatment plan.
