In this coordinator role, you will guide restorative nursing care across Rehab and Extended Care, collaborate with interdisciplinary teams on treatment goals, educate staff, strengthen evidence-based practices, and advance program improvements that enhance Veteran outcomes and continuity of care every day.
- Bring your nursing expertise to a role where you can shape restorative care and make a lasting difference in the lives of Veterans. As the Restorative Registered Nurse, you will serve as a key resource within Rehab and Extended Care, helping Veterans achieve and maintain their highest possible level of function and independence. You will partner with nurses, rehabilitation specialists, medical providers, community facilities, and other VA programs to coordinate individualized restorative services and strengthen continuity of care. This position offers the opportunity to educate and support staff, influence evidence-based practice, contribute to program development, and lead improvements that enhance the quality and effectiveness of restorative nursing services. Duties include, but are not limited to: - Coordinate restorative nursing services for Veterans in Rehab and Extended Care, supporting individualized treatment plans designed to promote function and independence. - Collaborate with interdisciplinary team members and treatment providers to evaluate Veteran needs, establish restorative goals, and integrate appropriate interventions into the overall plan of care. - Educate new and existing staff on restorative nursing treatment options, available resources, and practices needed to successfully implement and sustain the program. - Promote evidence-based restorative nursing interventions and recommend innovative approaches that improve care delivery and Veteran outcomes. - Serve as a clinical consultant and resource to nursing staff, rehabilitation services, medical providers, and other partners addressing restorative nursing needs. - Develop and update policies, procedures, memorandums, educational resources, and informational materials that support effective restorative nursing services. - Coordinate care with VA providers, community facilities, and other VA hospitals and clinics to strengthen continuity and access to appropriate restorative services. - Lead and participate in performance improvement activities, using program information, collaboration, and evaluation of current practices to strengthen restorative nursing services.
