- Summary The Registered Nurse-Ambulatory Care is accountable for all elements of the nursing process when providing and/or supervising direct patient care. The outpatient population is managed via Primary Care (Patient Aligned Care Teams). Duties include but not limited to: Providing competent, evidence-based care to assigned primary care patients in the ambulatory care clinic. Offering oversight and guidance to licensed practical nurses and health technicians within the clinic environment. Delivering direct patient care, including care coordination, health promotion, and education on wellness, disease prevention, and chronic care management specific to chronic care conditions. Assisting in the direction of nursing education, orientation, and competency evaluations for staff. Contributing to quality improvement initiatives and tracking outcomes utilization in the ambulatory care setting. Demonstrating leadership in delivering and enhancing holistic specialty care through collaboration with interdisciplinary teams. Evaluating clinical practices on an ongoing basis, drawing from best evidence to inform care decisions. Providing informal constructive feedback to peers to foster professional growth and improve care delivery. Creating a safe and supportive environment that promotes the professional development of healthcare team members. Contributing a professional nursing perspective in interdisciplinary team discussions related to patient care plans. Partnering with colleagues to drive change and achieve optimal patient outcomes in ambulatory care services. Supporting team members by sharing knowledge to ensure safe, high-quality ambulatory nursing care. Disseminating educational findings, experiences, and innovative ideas with peers to advance practice. Questioning existing clinical practices to promote evidence-based care in the outpatient setting. Participating in activities and strategies aimed at sustaining a culture of evidence-based practice within the clinic. Performing additional duties as assigned, which may include patient intake, documentation, point-of-care testing, medication reconciliation, and coordinating patient readiness for procedures.
