The primary purpose of the Licensed Practical/License Vocational Nurse (LPN/LVN) is to perform assigned duties in a manner that promotes quality health care, positive customer service and the maintenance of an environment committed to serving the Veteran. The Licensed Practical/License Vocational Nurse (LPN/LVN) will perform under frequent guidance of the Nurse Manager or designee. Assigned tasks require basic technical competence and skill.

  • The incumbent will be working at The Huntington Hershel "Woody" Williams VA Medical Center/Nursing Service/PACT facility. The LPN/LVN is responsible and accountable for individual nursing practice. The Patient Aligned Care Team (PACT) Licensed Practical Nurse (LPN) provides direct support to the PACT teams in Ambulatory Care Outpatient Clinics. The incumbent is accountable for greeting the Veterans, using different modalities of gathering Veteran data such as observation, vital signs, and recording information the Veteran provides them; communicating this data to the Registered Nurse (RN) and/or provider; and taking action as directed. They possess basic LPN skills and functions of a new or recently graduated LPN at a GS-3 level, having possess basic LPN skills with at least six months prior experience as an LPN at a GS-4 level , possess a higher level than basic LPN skills with at least one year prior experience as an LPN at a GS-5 level in a direct patient care environment. They assist in assuring smooth clinic flow by assisting the Veteran through the transition process as they move through the clinic environment. The LPN works under the direction of the Nurse Manager, Assistant Nurse Manager, or the RN Care Manager. The LPN/LVN GS-6 is expected to function at a fully successful level for all key elements at the GS-5 grade. The LPN/LVN GS-6 is a full performance functional level for the LPN/LVN occupation for providing practical nursing care with a variety of physical and/or behavioral problems.
