The Outpatient Staff Registered Nurse (RN) is responsible for providing competent, evidence-based outpatient care to assigned patients and oversight of licensed vocational nurses/nursing assistants, as appropriate to the setting. The incumbent of this position will be working on the Patient Aligned Care Team (PACT) at the Tomah VA Medical Center.

  • The Registered Nurse (RN) - Outpatient / Patient Aligned Care Team (PACT) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. The person in this role assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The RN - Outpatient (PACT) is also responsible for the following duties: Assisting in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization consultation. Demonstrating leadership in delivering and improving holistic, outpatient care through collaborative strategies with others. Evaluating practices in an ongoing process, based on best evidence. Providing peers with informal constructive feedback for improvement. Fostering a safe and supportive environment conducive to the professional development of healthcare professionals. Contributing professional nursing perspective in discussions with the interdisciplinary team. Partnering with others to effect change and produce optimal outcomes. Supporting colleagues through knowledge sharing to provide safe, quality outpatient nursing care. Participating in activities and strategies to sustain evidence-based-practice. Other duties, as assigned.
