About the Position: This position is located at the 55th Medical Group, Offutt Air Force Base in Nebraska. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
- Coordinate healthcare services, referrals, and resources across military and civilian providers to ensure timely patient care.
- Analyze complex medical, psychosocial, and administrative problems to recommend effective solutions and improve outcomes.
- Provide comprehensive assessments of patient health needs and develop individualized, patient-centered care plans.
- Assess patient progress, collaborating with multidisciplinary team and patient/caregiver to address concerns and guide decision-making.
- Educate patients, families, and the healthcare team on case management, treatment options, insurance benefits, and available community resources.
