As a Patient Safety Informatics Registered Nurse, you will serve at the intersection of nursing, patient safety, and electronic health record optimization, helping identify risks before they affect Veterans and transforming safety information into meaningful improvements. You will collaborate with informatics specialists, patient safety professionals, quality leaders, and national partners to evaluate emerging issues, strengthen digital workflows, share lessons learned, and support safe system use. This position offers a unique opportunity to influence organizational decision-making, contribute to electronic health record modernization, and advance a strong culture of safety throughout the Sioux Falls VA Health Care System. Duties include, but are not limited to: - Review and reconcile patient safety events, technical tickets, system change requests, and incidents reported through Joint Patient Safety Reporting, ServiceNow, Remedy, and related electronic health record systems. - Identify patient safety risks within electronic health record workflows and partner with the facility Informatics team to evaluate concerns and develop practical solutions. - Analyze safety information using systems thinking and end-to-end workflow review to identify contributing factors, emerging risks, and opportunities to strengthen the reliability of digital care processes. - Conduct retrospective and prospective risk analyses using approaches such as Root Cause Analysis, Healthcare Failure Mode and Effect Analysis, accident and close-call investigations, and ticket analysis. - Synthesize patient safety findings and communicate trends, risks, and recommendations to facility leaders, patient safety professionals, informatics partners, and the National Center for Patient Safety. - Develop and share site-level lessons learned and timely safety updates that help internal and external stakeholders recognize risks and apply effective mitigation strategies. - Collaborate with leadership on communication strategies for electronic health record modernization patient safety objectives, risks, and system expectations, while contributing clinical expertise through the Informatics Committee. - Educate end users and super users during electronic health record go-live and sustainment activities on patient safety responsibilities, safe-use practices, and informatics workflows.
