Rocky Knoll opened in August 1926 as the place Sheboygan County sent residents diagnosed with tuberculosis. Advances in treatment emptied that mission by the 1950s, so the county turned the building toward caring for seniors, and soon after the 1965 Medicare and Medicaid legislation it began serving Medicare-eligible guests. Three additions later, a two-story west building in 1972, a north building in 1992, and a short-term rehab wing in 2002, the county-owned facility is licensed to serve up to 149 residents.

The county's page for the center records certification under Medicare A and B, Medicaid, and Veterans Administration programs, and the center's licensed nursing work is in Plymouth.
