Registered Nurse-Case Manager (Community Care)

This Registered Nurse position is in Care in the Community at the Newington VA Healthcare Systems, located at the Newington, CT. campus. This position is full-time at 40 hours per week.

• The Case Management (CM) Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings. The CM RN assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. The CM RN coordinates care with multiple providers across all levels and sites of care. The CM RN addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. The CM RN also evaluates care and outcomes to ensure timely and appropriate provision of services. The CM RN will assist in directing the provision of nursing education, orientation, competencies and providing quality improvement and outcomes utilization. Practice includes directing other providers. Partners with the patient, family, and interdisciplinary team to promote effective and safe transitions across the health care continuum. The CM RN has fundamental experience with the situations in which they are ultimately expected to perform, integrating theoretical concepts, basic critical thinking skills, and technically skilled practice. The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. This position is in the Excepted Service. The salary is based on the grade and step approved for the selected candidate.
