This position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. • The Senior Social Worker - Care Coordinator conducts clinical activities as a member of a Behavioral Health Interdisciplinary Program (BHIP) team, including general and specialty clinics such as Trauma Specialty Program (TSP) and Substance Treatment And Recovery (STAR) Program. The incumbent is responsible for the coordination and continuity of care for Veterans receiving services throughout Outpatient Mental Health. Duties include but are not limited to: Provides a full range of psycho-diagnostic services which requires advanced and expert skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. Participate in discharge planning with other disciplines and will be responsible for ensuring that the discharge plans are executed in a manner that is timely and appropriate to promote care continuity. Conducts risk evaluation and treatment, including oversight of interdisciplinary documentation of treatment plans, for Veterans within BHIP, including those who are in the REACH Vet program and those with a High Risk for Suicide Patient Record Flag. Documents interventions and activities in patient's electronic health record and ensure appropriate hand-off communication and documentation when transferring patient care. Maintain familiarity and share responsibility with other team members for performance measures relevant to the BHIP team. Provide consultation/education to Veterans and families on VA resources (including self-management tools), community resources, Advance Directives and VA benefits. Act as a liaison between the VA and community resources, to include education and bidirectional communication. Provides wellness/prevention education and may facilitate patient/family educational bridging supportive sessions as appropriate. Works with the Local Recovery Coordinator and the Mental Health Senior Social Workers to engage Veterans at increased risk for suicide, including providing additional psychotherapy appointments as needed. Provides subject matter consultation to colleagues and trainees on the psychosocial treatment of patients treated in the specialty area, rendering professional opinions based on experience and expertise and role modeling effective practice skills. Participates in program planning and development, advising the supervisor of shifting trends with care coordination needs and caseloads and identifying the need for making recommendations on program procedures and/or policy changes based upon work experiences. Perform other duties as directed by the Supervisor, such as, but not limited to training/mentoring and oversight of new staff and/or those not yet clinically licensed and preparing for and training staff on Commission on Accreditation of Rehabilitation Facilities (CARF) and The Joint Commission standards/accreditation process.
