Licensed Professional Mental Health Counselor (Primary Care Mental Health Integration)

The Primary Care Mental Health Integration (PCMHI) Licensed Professional Mental Health Counselor (LPMHC) position is administratively aligned with the Mental Health Service and serves as a Collocated Collaborative Care (CCC) behavioral health consultant in Primary Care.

• The LPMHC provides functional assessment, triage, brief intervention, education, and consultation for a broad range of behavioral health and health behavior concerns. Major duties include but are not limited to: Collaborating with Primary Care Providers and interdisciplinary PACT members to provide high-quality, coordinated care. Conducting focused functional assessments and providing brief, evidence-based individual or group interventions. Maintaining open-access appointments and encouraging same-day warm handoffs within PACT. Utilizing Measurement Based Care and stepped-care principles to match services with Veterans' level of clinical need. Referring Veterans with more severe or complex needs to appropriate general or specialty mental health programs. Providing consultation and education to PACT members regarding behavioral health, health behavior, communication, motivation, and behavior change. Completing timely clinical documentation and providing appropriate feedback to Primary Care Providers and other PACT members. Participating in counseling administrative initiatives, performance improvement activities, training, education, and program evaluation activities as assigned.
