Social Worker Program Coordinator-Veteran Directed Care

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

• Total Rewards of a Allied Health Professional Establishes and maintains effective working relationships and communicates with Veterans, staff, and other parties as appropriate. The incumbent will: Perform and oversee consultation and facilitation of agreements in coordination with the Office of Community Care for VDC. Develop, coordinate, select, and evaluate VDC in the WCHCS catchment area. Implement and apply Veteran's Health Administration (VHA) guidelines to ensure standards maintained promote health, safety, welfare, and satisfaction of the Veteran receiving community services; and appropriate oversight to ensure compliance with all VHA guidelines. Utilizes knowledge of community resources, makes referrals to the community and other government agencies as appropriate, and coordinates services. The incumbent will: Develop and provide any program reports to VISN and VA Central Office (VACO) for the Program and seek guidance from VACO program office, as needed. Perform a wide range of services and provide technical assistance to State, local, and private agencies. Independently conducts psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. The incumbent will: Participate in direct Veteran and caregiver contacts to assess caregiver stress/burden and the development/application of directed clinical treatment interventions. Provides consultation, education, and advocacy. The incumbent will: Work with the partnering agencies to complete Readiness Reviews, provide consultation, and monitor of programmatic progress. Utilizes a range of specialized interventions and treatment modalities in treatment or with specialized Veteran populations, including individual, group, and/or family counseling or psychotherapy, and advanced level psychosocial and/or case management. The incumbent will: Learn current theory and treatment modalities by reading professional journals, attending conferences, clinic seminars and teleconferences, and materials distributed through VA and private sector sources. Incorporates complex multiple causation in differential diagnoses and treatment within approved clinical privileges. The incumbent will: Independently conduct psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds. Utilizes knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. The incumbent will: Ensure policy and procedural compliance with the VDC program. Utilizes knowledge and ability to write policies, procedures, and/or practice guidelines for the program. The incumbent will: Create educational tools, develop programs, and implement training to VAMC employees. Demonstrates skills in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program areas. The incumbent will: Provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs, as assigned..
