This position is eligible for the Education Debt Reduction Program, a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. • Duties may include but are not limited to: - The Community Living Center/Hospice and Palliative Care (CLC/HPC) full-time physician is assigned to provide medical care to patients on the Community Living Center (40-bed unit with Rehabilitation, Skilled Nursing, Respite, Hospice, and Dementia cohorts) in collaboration with two midlevel Nurse Practitioners (NPs), as well as palliative and hospice care services in consultation with the Palliative Care Consultation Team (NPs, Registered Nurse [RN], Social Work [SW], Pharmacist [PharmD], and Chaplain) as needed for cross coverage. - The full-time physician works under the direction and oversight of the Associate Chief of Staff for Geriatrics and Extended Care (ACOS/GEC). - Participates in screening calls and completes chart reviews on all referrals to the Community Living Center to ensure patients referred are medically stable. Communicates with referring physicians if questions arise. Collaborates with other disciplines such as Nursing, Physical Medicine and Rehabilitation (PM&R), Social Work (SW), etc., to ensure all aspects of patient care can be provided on the Community Living Center. - Assists in discharge planning to ensure effective transfer of patients from acute care to the Community Living Center, Community Nursing Home (CNH), or discharge home with community hospice services, as necessary. - Ensures "hand-off" communication is provided to the Emergency Department (ED), Hospitalist, Surgeon, Neurologist, or University of Kentucky (UK), if a resident is sent out of the Community Living Center for an acute care evaluation. - Directly evaluates all residents with any change in condition, acute event, or as requested by any Interdisciplinary Team (IDT) member, and documents this evaluation in a timely manner, on the same day. - Completes attending notes for Community Living Center residents with significant changes in clinical condition, as well as follow-up (progress) notes as needed after unit rounds. Reviews labs and imaging promptly and takes action if abnormal. VA offers a comprehensive total rewards package. VHA Physician Total Rewards.
