This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Disease processes include, but are not limited to: diabetes, hypertension, hyperlipidemia, chronic obstructive pulmonary disease, mental health disorders, comprehensive women's health care, etc., with a major focus on preventive medicine. Lead the Patient Aligned Care Team (PACT) approach in the delivery of care, which emphasizes a team-based approach to healthcare. Document all medical information in an electronic medical record. Perform medication reconciliation. Provide effective delivery of outpatient care while meeting and exceeding compliance standards. Maintain current clinical privileges, licensure, Basic Life Support (BLS), and all other requirements as appropriate. Cooperate with, and participate in, medical duties or functions related to quality of care and peer review along with improvement/performance improvement activities at the direction of the Chief of the Medical Service. Work Schedule: FT, Monday - Friday, 8am to 4:30pm
