The ACOS of PC will also serve as a Primary Care physician in the PC Service according to approved clinical privileges at VA PIHCS with an expected average clinical time of at least 0.1 FTEE per week. The position is located Honolulu, Hawaii. The incumbent reports to the COS and serves as a key clinical resource for the Health System Leadership. Duties include but are not limited to: Leadership/Supervision Providing direction and leadership of the Patient Aligned Care Team (PACT) model that ensures facility's goals and benchmarks for integrated care delivery are met. Supervises a variety of positions directly, or indirectly (administrative, physicians, nurse practitioners, and other clinical staff as assigned by the Chief of Staff). Responsible for the management and oversight of clinical and administrative operations in Primary Care. Provides physician/clinical/administrative leadership and serves as an advisor to Chief of Staff and other service line leaders. Responsible for timely completion of all program evaluations and documents required by the Medical Center, VISN, and VACO, ensuring compliance with appropriate bodies (such as TJC and Ascellon) and monitoring entities as appropriate. Has supervisory responsibilities noted above, with additional responsibilities including management of Human Resource, performance appraisals, and professional boarding procedures for various disciplines. Support all existing safety and infection control practices relevant to this position. Contributes to and keeps abreast of new safety and ergonomic practices as they are developed. Complies with safety, security, and privacy policies, arranges proper clinical coverage during planned absences, and complies with hand-off communication policy. Quality Improvement Directs the continuous measurement and improvement of the quality, timeliness and patient satisfaction with primary care provided by the service. Targeting performance improvement initiatives to address national programs such as SAIL (Strategic Analytics for Improvement and Learning), HEDIS (Healthcare Effectiveness Data and Information Set), productivity, and utilization Management/Efficiency of care. Plans, coordinates, and ensures high quality, timely, patient centered care through utilization of multiple quality measurement tools. Improves patient outcomes through the implementation of strategies such as clinical practice guidelines and critical pathways, health risk assessments, provider profiling, use of electronic medical record and clinical reminders, and case management. Continuously assesses and improves the processes of primary care delivery in the service so as to enhance patient safety. Provides continuing surveillance of the professional performance of all providers who have delineated Primary Care clinical privileges. Budget and Fiscal Management Provides active strategic decision-making and organizational planning, to include budgetary and financial oversight. Collaborates with other service chiefs in the development and execution of the overall Medical Center strategic and business plan. Plans initiatives and develops programs in collaboration with other services in order to ensure coordination of service, reduction of redundancies, and enhancement of cost effectiveness.
