SUMMARY:    Responsible and accountable for creating, implementing, and facilitating patient discharges and transfers from the acute care setting. The Social Worker collaborates with the patient and caregiver, the patient’s care team and outside entities to ensure safe and timely patient discharges and transfers

BAXTER HEALTH COMPLIANCE RESPONSIBILITIES

Understands and adheres to Baxter Health standards and polices.

 Required to work with the Director regularly to carryout, initiate and/or implement best practices for the Baxter Health Continuity of Care Department.

 

JOB REQUIREMENTS

Education: Master’s Degree in an accredited Social Work program.

Licensure: Current Arkansas Social Work license in good standing.

Experience: 12 to 18 months related experience and/or training or equivalent combination of education and experience.

Other: Ability to read, write and communicate effectively. Proficient with Microsoft Word and Excel.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job with or without an accommodation.  While performing the duties of this job, the employee is required to perform the following:  Must have the ability to communicate effectively, orally and in writing, to solve problems and make decisions.  Spend 8 hours or more in front of computer, monitor or similar screen utilizing keyboard and/or mouse, daily. Ability to stand and walk, bend, stoop and twist, and occasionally lift up to 20lbs.  Acuity to the five senses is required in contact with patients, family members, providers, volunteers and co-workers.  Must be able to handle critical and highly stressful situations with efficiency and composure.

Office setting, within a hospital environment

 

Position Type and Expected Hours of Work

This is a position in a hospital setting which is open 24 hours a day, 365 days a year.

Social work staff: 5 days a week, Monday – Friday 8 hour shifts with rotating Saturdays.
