The Social Worker in this position is to function as a GS-12 HCHV Outreach Social Worker in Community Engagement and Reintegration Services (CERS) and performs services as a member of the Health Care for Homeless Veterans (HCHV) Street Outreach Team.

Street outreach also encompasses engaging Veterans who are at-risk for homelessness and Veterans who are residing in non-VA communal settings, such as community shelters. As a member of the HCHV Street Outreach Team, the incumbent will provide direct patient care, in a primarily field based capacity, to homeless Veterans with serious mental illness, substance abuse, and medical issues in communities throughout the Greater Los Angeles Veterans Affairs Healthcare Systems (GLAVAHS) catchment area (Los Angeles County, Kern County, Santa Barbara and Ventura Counties). S/he is responsible for assisting with patient eligibility and enrollment, providing linkage and support, and community health or mental health providers and agencies that serve homeless populations. The incumbent provides education, collaboration, and support to community agencies regarding VA homeless programs, benefits, and medical/mental health services. Work Schedule: Full-time, Monday-Friday, 7:00 am-3:30 pm Telework: Ad hoc may be available after 90-days Virtual: This is not a virtual position. Functional Statement #: 000000 Physical Requirements: Light lifting (under 15 pounds); light carrying (under 15 pounds); reaching above shoulder; use of both fingers' both hands required; walking; standing; hearing (aid permitted). Traveling throughout the medical center and the community is requires, as is performing activities involving sitting, walking, standing, bending, and carrying such items as books, papers, and files. In carrying out responsibilities it will be necessary for the incumbent to travel into the community where he/she conducts, interviews with Veterans, their families, representatives of the community health, welfare agencies and law enforcement agencies.
