The Clinical Pharmacist is a licensed professional responsible for providing pharmaceutical care to optimize patient response and resource utilization for professional staff at Bob Stump Department of Veterans Affairs Medical Center in Prescott, Arizona.

  • Typical duties include but are not limited to: Non-VA Prescription Support Receives faxed, mailed, electronically prescribed, or delivered prescriptions written by non-VA providers and documents all received prescriptions in CPRS. Reviews Community Care Referral and Authorization site and CPRS to determine authorization of non-VA Care. Reviews authorized prescriptions for appropriateness using pharmacist knowledge, as well as knowledge concerning the national drug formulary and local VA formulary requirements. Assists in the tracking and shipping of non-VA prescribed controlled substances, as well as all non-VA prescribed medications mailed from the Consolidated Mail-Out Pharmacy (CMOP). Controlled Substance Non-VA Care Prescriptions Reviews the Arizona Prescription Drug Monitoring Program (PDMP) website to ensure appropriateness of received prescription, determine exact due date of medication, and prevent duplicate and overprescribing of controlled substances and document in CPRS. Maintains a current knowledge of national, state, and local laws, regulations, directives, policies, and procedures pertaining to controlled substances. Secures controlled substances while filling Non-VA Care controlled substance prescriptions. Documents all controlled substance entries through the computerized controlled substance package perpetual inventory system. Monitors profiles for abuse potential and notifies the Mental Health or Pain Clinical Pharmacist Practitioner if discovery is made. Telephone Triage Utilizes pharmacy information systems to answer patient or provider questions related to status of their prescription(s), completion date, package tracking, etc. Documents all phone conversations in CPRS for future reference. Functions as a resource person to members of the pharmacy, medical center, and CBOCs regarding non-VA Care Prescriptions. Clinical Provides appropriate selection of drug therapy based upon the pharmaceutical principles of pharmacokinetics and pharmacodynamics; monitoring for efficacy, side effects and clinical outcome; and advises prescribers, as appropriate. Provides patient-specific therapeutic drug monitoring and communicates relevant findings and / or recommendations to other health care providers in charge of the patient both orally and in writing. Evaluates pertinent laboratory data, drug-drug, and drug-nutrient interactions, monitors for adverse drug effects, and screens for allergies. Performs medication reconciliation at all transitions of care, including updating the medication profile to reflect an accurate, active list of VA and non-VA medications. Reports adverse drug events (ADE), near misses, and medication errors in alignment with VA ADERS reporting program. Education Provides education to health care providers regarding medication use. Effectively instructs patients and family members in the appropriate use of medications and medical devices. Maintains basic knowledge of pharmaceutics, pharmacokinetics, pharmacodynamics, and pharmacotherapeutics in practice. Ability to communicate orally and in writing to both patients and health care staff. Possesses knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security.
