The Community Health Worker works to improve access to health care services; increase screening; build understanding between community members and the health and social service system; enhanced communication between community members and health providers. CHW provides social support and informal counseling, advocates for individuals and community health needs.
- Improve access to prenatal and postnatal health care services.
- Responsible for establishing trusting relationships with participants and their families while providing general support and encouragement.
- Effectively communicate the Perinatal and Family Center Initiative (PFCI) mission.
- Provide ongoing follow-up, motivational interviewing, and goal setting with participants/families.
- Follow–up with participants via phone calls, home visits, and visits to other settings where participants can be found.
- Assist participants in understanding perinatal care plans and instructions.
- Provide referrals for services to community agencies as appropriate.
- Complete required reports reflecting program services.
- Manage assigned caseload of participants and work collaboratively and effectively within a team.
- Work closely with OB-GYN medical providers and Doulas to help ensure that participants have comprehensive and coordinated care plans.
- Work collaboratively with other clinical personnel assigned to the same patient to communicate community resources appropriate to the needs of participants/families.
- Act as a patient advocate and liaison between the patient/family and community service agencies.
Bachelor’s degree in education, social, behavioral, or biological sciences or a directly related field from an accredited college or university and two (2) years of public health education or one (1) year of experience at the lower level Hlth Educator 2 (HCP031) or position equivalent.
PREFERRED QUALIFICATIONS:
- Four years of progressively complex office or administrative experience
- Experiencing collecting and analyzing data
- Ability to multitask
- Ability to work in a team environment
