PURPOSE AND SCOPE: Supports the mission, core values and customer service of Home Dialysis Services LLC (HDS). Adheres to the Home Dialysis Services Policies and Procedures and Compliance Program including all state and federal regulatory requirements. The Social Worker provides Social Services to patients to maximize the patients’ well-being and improve outcomes.
