This position is eligible for the Education Debt Reduction Program (EDRP) a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. - This position provides Substance Use Disorder (SUD) case management and intervention support to the Health Care for Homeless Veterans (HCHV) Program in a team oriented, recovery-based program. The Senior Social Worker provides on-going substance use case management and early treatment interventions to promote sustained stability and abstinence in homeless and at-risk Veterans receiving VA homeless services. Duties include but are not limited to: Provides direct SUD services for Veterans engaged in homeless programs of the Health Care System and rural regions of the catchment area Provides outreach services in the community and rural regions of the catchment area. Develops comprehensive treatment plans with the Veteran that address the factors that contributed to homelessness, including SUD and other co-occurring factors within a recovery model framework. Works with Veteran to develop treatment goals and quarterly reassesses and revises goals as indicated by goal completion. Organizes and conducts individual and group case management and SUD treatment, to include early interventions and supportive follow-up to ensure stability in home visits and facility-based contacts. Provides SUD support to the homeless programs team as a functional member, to include consultation, interventions, or accompanying other case managers on home visits, when indicated. Serves as a member of the multidisciplinary homeless program treatment team and the STAR program. Develops partnerships with community agencies, especially those serving homeless and SUD Veterans, through regular contact and communication. Provides liaison services with other programs referring Veterans to homeless programs services to ensure continuity of care and to promote successful transition of Veterans between programs. Participates in policy development, goal setting, and treatment service planning discussions with other members of the homeless program team. Collects and reviews statistical data to improve performance and delivery of services to homeless Veterans. Provides recovery-oriented treatment with a goal of independently living in the community at the Veteran's highest level of functioning and quality of life. Provides oversite of the services provided by HCHV Contract Residential Program, including assisting with quarterly and annual reviews/inspections of program operation, as well as coordinating services for Veterans engaged in the program. Provides clinical case management and crisis intervention services to Veterans with complex SUD histories who are engaged in the HUD-VASH program.
