This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible.

• The Clinical Pharmacy Specialist (CPS) -Primary Care PACT functions as a member of their assigned PACT Teams. The CPS works independently under their individual scope of practice to provide direct patient care. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy. The CPS primarily works at the James J. Peters VA Medical Center but may be asked to work at any of the community based outpatient clinic sites if needed. Duties include and are not limited to: Serves as an independent practitioner in the ambulatory setting requiring the highest level of pharmacist clinical privileges with minimal supervision. Performs administrative, direct patient care, and non-direct patient care activities. Utilizes different modalities to provide care as per facility guidelines including telehealth appointments. Participates in medication utilization evaluations and other quality assurance activities to ensure appropriate and safe medication use. Directly assists other providers in rational medication selection and monitoring by providing written and/or verbal consultation. Participates in formal and informal educational programs of the service, including serving as a role model and preceptor to pharmacy students and residents.
