This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.
- Total Rewards of a Allied Health Professional Duties include but are not limited to: Provide timely screening, comprehensive psychosocial assessments and regular reassessments to determine clinical needs, and diagnoses of mental disorders. Provide outreach services to engage justice-involved Veterans.- Verify Veterans' status, eligibility for VA healthcare, and eligibility for VJO case management participation. Accept referrals for screening and admission into V JO case management. Function as a clinical case manager for Veterans who are justice-involved, performing duties to facilitate independent community living by coordinating health, mental health, and community care. Collaboratively develop treatment plans with Veterans enrolled in VJO case management. Monitor progress, review and update regularly throughout the episode of care. Provide a full range of services within the Social Work Scope of Practice based on Veteran needs, acuity level, and preferences for care. Services may include, but are not limited to, the following: case management; individual, family, and group therapy; discharge planning; resource linkage; and care coordination. Provide consultation and psychoeducation to Veterans and Veteran families on topics including community resources, Advance Directives, VA benefits, budgeting, activities of daily living, and recovery goals. Collaborate with VA multi-disciplinary teams and community agencies to develop effective and efficient ways of delivering healthcare services to justice-involved Veterans through treatment coordination and communication.
