About the Position: This position is located at Scott Air Force Base, in St. Clair County, Illinois. This is a Direct Hire Solicitation A recruitment or relocation incentive may be authorized. Salary negotiation may be available for those candidates who are new to Federal service. Student Loan repayment may be authorized. Non-Federal work experience may be creditable for leave purposes.
- Perform independent, comprehensive telephone and clinical triaging in the primary care setting, supporting primary care providers in their treatment of patients with mental health conditions.
- Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools.
- Communicate with providers regarding patient status and documents appropriately in the electronic medical record.
- Coordinate patient referrals to support services provided through the internal and external community, or the TRICARE network.
- Compiles and maintains a variety of data reports, submitting detailed and aggregate program and caseload data reports as needed by program staff.
